| Placement Details |
| 1. |
Employee shall
receive an annual salary commensurate with the prevailing wage rate, payable
weekly. For the purposes of this Agreement, “prevailing wage’(also
known as the “going rate”) shall be defined as the wage then
being paid by healthcare facilities within the same geographic area as employee
has been placed, for the work being performed by Employee. |

| Pay rates for general RN are
$25 an hour and for a specialty RN $28 an hour. A 3% increase will
be provided per year |

| 2. |
Employee must
be available to work for a minimum of 40 hours per week. The forty hours
per week will be guaranteed by the employer. Employee will be paid time-and-half
for hours worked over forty hours in one week. |
| 3. |
Employee shall
become eligible to enroll in Employer’s hospitalization program after
three months of employment provided that the above hours are maintained.
The employee may call in “sick day” but must make up for the
time without extra pay. These employee benefit plans are subject to change
by the employer at any time without notice. |

Employee is
eligible for annual performance reviews which may entitle employee to salary
increases. |
| 5. |
Prior to beginning
work, employee, at his/her sole cost, and expense, must attain, and must
there after maintain throughout the term of this agreement, a physical,
CPR, and malpractice insurance. |
| 6. |
Employee must
pre-register for NY and NJ (RN and LPN) State boards four (4) months prior
to arrive in the U.S. |
| 7. |
Employer will
pay for employee’s attorney’s fees required to obtain a green
card, one way airfare to the U.S. (up to $1,000), visa screen certificate,
and provide, at a location of employer’s sole discretion, three (3)
months of free housing. Employee will be responsible for all other fees
necessary to obtain sponsorship. |
